Maiden Surname:

Enter your last name as it appears on your birth certificate.

Street Address:

Enter the street address of your usual residence, even if you plan to move after the ceremony. P.O. boxes are not accepted.

County of Residence:

If you live in Virginia, select the county where you currently reside from the drop down menu. If you do not live in Virginia, manually enter the name of the county where you reside. If you live in an independent city (regardless of the state) or you live outside the United States leave this field blank.

Elementary/Secondary School Attendance:

Select the number of years you attended elementary and/or secondary school from the drop down menu. 12 years is the equivalent of finishing high school or a GED program. If you attended school in a different country, select the number of years you attended school in that country. If you graduated from school in a different country, select 12, regardless of how many years you attended school.

College Attendance:

Select the number of years you attended college from the drop down menu. If you did not go to college, leave this field blank. If you received an associate’s degree select 2, regardless of how long it took to receive the degree. If you received a bachelor’s degree, select 4, regardless of how long it took to receive the degree. If you went to graduate school, law school or medical school select 5+.

Parent's Name:

Enter each of your biological parents’ full name (first, middle, last and suffix) as it appears on your birth certificate. If either of your parents has a maiden name that is different from their married name, do not include their married name. Enter their full maiden name. It is important to spell your parent’s name correctly. Include your parent’s full middle name. If your parent is deceased, still include their full name. Do not include a step-parent. Do not leave this field blank.
If your parent is on your birth certificate, you must include their name on your license. If your parent is not on your birth certificate, entering their name is optional. If you don’t want to include their name or you don’t know who your parent is, enter “unknown.” If you were legally adopted, enter the full name of your adoptive parent, exactly as it appears on your birth certificate. If your parent legally changed their name by court order, then list their new name as specified on the court order.

Previously Married:

If this is your first marriage, select “No.” If this is not your first marriage select “Yes.”

Number of this Marriage:

You must include the number of times you have been married on the marriage license. Using the drop down menu, including this marriage, select the number of times you have been married. For example, if this is your second marriage, meaning you were previously married once before, select “second.” If this is your third marriage, meaning you were previously married twice before, select “third.”

Marital Status:

Select how your most recent marriage ended.
